3. Injury Reporting and Hockey Accident Insurance


INJURY REPORTING AND HOCKEY ACCIDENT INSURANCE

Injury Reporting
England Hockey requires data on injuries sustained across the sport so that trends can be identified and reported to both England Hockey’s Board and the International Hockey Federation (FIH). It is imperative that England Hockey collects this data in order to influence rules changes and improve the safety of the sport.

In order to collect meaningful data, England Hockey requires clubs/associations to complete the England Hockey online ‘Injury Monitoring Form’ following injuries sustained that require one or more of the following:

  • Treatment from a first aid specialist (e.g. Team doctor/ first responder)
  • Hospital treatment
  • Subsequent visit(s) to a GP

The reporting of injuries sustained on the field of play is a requirement for all clubs/ organisations who are affiliated with England Hockey as part of England Hockey’s Injury Reporting Policy.

Please complete this form for any injuries sustained which meet one or more of the criteria outlined above.

England Hockey – Personal Accident Insurance
Commencing on 1s September 2021, the club has entered into the new England hockey insurance arrangements, which includes personal insurance for members participating in hockey.

Personal Accident cover provides a set financial payment in the event of an accidental injury, as detailed in the schedule of benefits below, whilst playing or practising the sport of Hockey, including travelling to and from any such activity.
The benefits provided are as follows:
• Death or Permanent Total disablement £75,000 (Under 16 years £10,000).
• Loss of limbs/eyes, up to £75,000.
• Broken bones (arms/legs) £300.
• Optical Treatment £1,000.
• Dental Treatment £5,000.
• Physiotherapy up to £1,000.
• Temporary total disablement (per week) £300 per week.
• Concussion £500.
• Scarring/facial disfigurement £750.
• Paraplegia £75,000.
• Quadriplegia £75,000.
• Coma benefit (per day) £100.
• Hospitalisation (per day) £50.
• Reimbursement of membership fees £500.

What should be reported?
Any injury likely to lead to a claim, which is likely to be any incident requiring medical intervention (on the day or subsequently) is reported (in line with the separate compulsory EH Injury reporting above).
